If scalp hair is not present, an equivalent quantity of body hair may be used. Reference to head hair implies scalp hair, while body hair encompasses all other types such as facial and underarm hair.

Process Overview

The laboratory processing of drug test outcomes entails four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary step in incorporating a sample into the lab's system. This process ensures the sample was sealed and transported correctly, generates a random LAN (Laboratory Accessioning Number), and completes any necessary data input absent in an electronic chain of custody.

Screening serves as the preliminary stage for drug of abuse detection. While Screening is economically beneficial for eliminating drug use across most samples, a positive result necessitates confirmation to be court-admissible. Samples indicating a presumptive positive in Screening are subject to further confirmation.

If a sample shows a presumptive positive during Screening, additional hair is extracted from the initial sample for the Extraction process. During this phase, drugs are removed from hair at a lower concentration than alternative methods (e.g., urine or oral fluid), making hair drug analysis the most complex methodology.

Any positive result from Screening is verified through GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positive results are washed before Confirmation if necessary. The complete lab process, from Accessioning to Confirmation, is examined under the CAP (College of American Pathologists) Hair certification and adheres to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
