Collecting a sample of 1.5 inches of about 200 hair strands (resembling the diameter of a #2 pencil) nearest to the scalp yields around 100mg of hair, which is the optimal amount for both screening and verification. For EtG, additional analyses, and/or tests above 10 panels, 150mg of the sample is advisable. It is best to measure the sample using a jeweler's scale. In instances where scalp hair is not attainable, an equivalent amount of body hair can be collected. The term head hair specifically means scalp hair, while body hair encompasses all other types (such as facial and axillary hair).

Process Overview

The key steps in the lab processing of a drug test result consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample within a lab's system. Tasks include confirming the sample was sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any remaining data entry not covered by an electronic chain of custody system.

Screening involves a preliminary assessment for drugs of abuse. Although Screening is an economical method for excluding drug usage in the majority of samples, a positive screen needs verification for legal acceptability. Any samples that test presumptively positive in Screening require follow-up confirmation.

For samples testing presumptively positive during Screening, more hair is extracted from the original sample and readied for Extraction. Here, drugs are retrieved from hair at significantly lower concentrations than with other techniques (like urine or oral fluid), which makes hair drug screening the most challenging technique to conduct.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
