What it is, How it works

Hair testing is recognized as an effective method for detecting drug and alcohol consumption. Hair serves as a long-term record of alcohol and various drugs by capturing biomarkers within the strands as they grow. When obtained near the scalp, hair can offer a detection period of up to roughly 3 months for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and convenient to transport.

A 1.5-inch section comprising approximately 200 hair strands (resembling the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal sample size for both screening and confirmation. For EtG tests, additional tests, and tests exceeding 10 panels, a specimen of 150mg is advisable. It is suggested to measure the specimen using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be used. When mentioning head hair, we are exclusively referring to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of processing a sample into a lab’s system. This process entails verifying that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry that might not be provided by an electronic chain of custody system.

Screening entails an initial rapid check for the presence of drugs of abuse. While screening is a cost-efficient way to exclude drug usage in most samples, a positive screening result requires confirmation to be admissible in court. Samples that are presumptively positive in Screening necessitate secondary confirmation.

If a sample is presumptively positive during the Screening phase, additional hair is retrieved from the initial sample and prepped for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Confirmation of any positive screening result is conducted using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The complete laboratory process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.
