Yet, urine tests remain most effective for capturing use within the past 5 days.

What it is, How it works

Hair testing is recognized as an effective means for detecting drug and alcohol consumption. Hair offers an extended history of substance use by capturing biomarkers within the fibers of the developing hair strand. When gathered near the scalp, hair can present nearly a 3-month detection window for alcohol and other substances. Hair collection is straightforward, relatively tamper-resistant, and convenient for transportation.

A 1.5-inch sample consisting of about 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, considered the optimal sample for screening and verification. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the sample is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent amount of body hair can be used. Reference to head hair concerns only scalp hair. Body hair includes all other hair categories (facial, axillary, etc.).

Process Overview

The primary stages involved in laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab's system. This process confirms that the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data not included by an electronic chain of custody system.

Screening involves an initial brief assessment for drugs of abuse. While screening is an economical method to exclude drug usage in most samples, a positive result must be confirmed for legal acceptability. Samples showing presumptively positive in screening require secondary confirmation.

If screening indicates a presumptively positive result, additional hair is collected from the initial sample for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample is washed as necessary before confirmation. The full laboratory process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 criteria.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
