What it is, How it works

Hair testing is increasingly recognized as an effective method for detecting drug and alcohol consumption. Hair captures a long-term record of alcohol and drug intake by trapping biomarkers within the strands as they grow. When collected near the scalp, hair can reflect substance use over roughly a 3-month period. Hair collection is straightforward, challenging to tamper with, and simple to transport.

A hair sample of 1.5 inches taken from about 200 strands (about the diameter of a #2 pencil) nearest the scalp provides 100mg of hair, which is optimal for evaluation and verification. For EtG, additional tests, and/or panels beyond 10, it is advisable to use 150mg of the sample. Using a jeweler's scale for weighing the sample is suggested. If scalp hair isn't available, an equivalent quantity of body hair may be used. References to head hair pertain solely to scalp hair. Body hair encompasses all other types (facial, underarm, etc.).

Process Overview

The laboratory processing of a drug test result primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into a lab's system. This step includes checking that the sample was sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and performing any required data entry not covered by an electronic chain of custody framework.

Screening is a preliminary quick assessment for drug abuse. Although Screening is an economical way to eliminate drug use for most samples, a positive result needs validation to be court-admissible. Samples that indicate a presumptive positive in Screening must undergo a secondary confirmation.

If a sample produces a presumptive positive during Screening, additional hair is extracted from the original sample and prepared for Extraction. Here, drugs are isolated from hair at much lower concentrations than other techniques (for instance, urine or oral fluid), making hair drug testing a particularly challenging method to conduct.

Positive Screening results require Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The complete lab process, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair designation and with accreditation to ISO / IEC 17025 standards.

Note: Although termed "hair follicle tests", the examination targets the hair strand rather than the follicle beneath the scalp.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
