Hair testing is regarded as an effective method for spotting drug and alcohol consumption. Hair captures a prolonged record of alcohol and drug use by sealing biomarkers in the hairs as they grow. When clipped near the scalp, hair offers a detection window of roughly up to 90 days for alcohol and drugs. Hair is easy to obtain, somewhat hard to alter, and simple to transport.

A sample measuring 1.5 inches that includes about 200 hair strands (similar to the thickness of a #2 pencil) that is near the scalp yields 100mg of hair, which is the preferred sample size for screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale to weight the specimen. In the absence of scalp hair, a comparable volume of body hair might be used. By head hair, we mean scalp hair alone. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The process of laboratory handling of a drug test result involves four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial entering of a sample into the lab's system. This consists of confirming the sample was properly sealed and dispatched, designating a random LAN (Laboratory Accessioning Number), and completing any further data entry tasks absent from an electronic chain of custody protocol.

Screening entails a preliminary rapid check for drugs. While Screening offers a cost-effective approach to eliminate drug use on most samples, a positive result requires confirmation to stand up in court. Any indicators of drug use identified in Screening call for further confirmation.

Should a sample show a positive during Screening, extra hair is extracted from the initial set and made ready for Extraction. In this phase, drugs are removed from hair at a concentration lower than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to perform.

Verification of any positive screen result occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples are cleaned before confirmation if necessary. The full laboratory process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditations.
