What it is, How it works

When taken near the scalp, hair can reveal drug and alcohol consumption for up to roughly three months. The collection of hair is straightforward, relatively resistant to tampering, and can be easily transported.

A 1.5-inch portion containing around 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for both screening and verifying. For EtG, supplementary tests, and/or more than 10-panel tests, it is advised to use 150mg of the sample. We propose using a jeweler’s scale for precise weighing. Should scalp hair be missing, a similar amount of body hair may serve as an alternative. The term "head hair" specifically means scalp hair only. "Body hair" includes all other forms of hair (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with integrating the sample into a laboratory’s database. This stage ensures the sample was appropriately sealed and delivered, assigns a random LAN (Laboratory Accessioning Number), and fills in any missing data not included in an electronic chain of custody.

Screening performs an initial quick assessment for drugs of abuse. While Screening is cost-efficient for dismissing drug usage among most samples, a positive outcome must be verified to hold up in court. Any samples flagged as presumptively positive during Screening must undergo further confirmation.

Should a sample be marked as presumptively positive during Screening, additional hair is extracted from the sample and readied for Extraction. In this stage, drugs are extracted from hair in much lower concentrations than in alternate methods (for instance, urine or oral fluid), highlighting the complexity of hair drug screening as a testing method.

Confirmation of any presumptive positive result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All specimens classified as presumptively positive are cleansed before confirmation as required. The entire laboratory procedure from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.
