A 1.5-inch portion of approximately 200 hair strands (around the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the ideal quantity for screening and verification. For EtG, supplementary tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest weighing the sample on a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair may be procured. When mentioning head hair, we specifically mean scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The primary four stages involved in the laboratory analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a lab’s system. This process includes verifying that the sample was securely sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening consists of a preliminary rapid check for drugs of abuse. Although Screening is a cost-effective method to exclude drug usage in the majority of samples, a positive screen must be confirmed to be admissible in court. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample is presumptively positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are drawn out from hair at a significantly lower concentration than in other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as needed. The entire laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
