For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the sample is suggested. Using a jeweler’s scale for weighing the sample is advised. If scalp hair is not present, an equivalent volume of body hair may be taken. "Head hair" refers exclusively to scalp hair. "Body hair" pertains to all other hair varieties (e.g., facial, axillary).

Process Overview

The four critical phases in the lab analysis of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a lab’s infrastructure. This step includes ensuring the sample was appropriately sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not covered by an electronic custody chain.

Screening involves an initial, rapid assessment for substances of abuse. While Screening is an economical approach to dismiss drug presence in the majority of cases, a positive screen requires confirmation to hold up in legal settings. Presumptively positive samples from Screening mandate a subsequent verification.

For samples indicated as presumptively positive during Screening, additional hair from the original sample is utilized for Extraction. Here, drugs are drawn from hair at markedly lower levels compared to other methods (such as urine or oral fluid), which positions hair drug screening as the most intricate process to conduct.

Any confirmed positive from the screening stage is verified through GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptively positive sample undergoes washing prior to confirmation as needed. The entire lab process from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair specifications and adherence to ISO / IEC 17025 standards.
