What it is, How it works

Hair encapsulates long-term markers of substances by embedding biomarkers within the structure of the growing hair strand. When harvested proximal to the scalp, hair can offer a detection window of up to about 3 months for alcohol and other substances. Collection is straightforward, relatively tamper-proof, and easily shippable.

A 1.5-inch cut of approximately 200 strands of hair (roughly the diameter of a #2 pencil) nearest the scalp provides 100mg of hair, considered optimal for screening and validation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advisable. It's suggested to use a jeweler's scale for specimen weighing. If scalp hair is not available, an equivalent amount of body hair might be used. By head hair, we mean strictly scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial setup of a sample into a lab’s system. This consists of authenticating the sample’s seal and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry not covered by an electronic chain of custody system.

Screening serves as an initial cursory inspection for substances of abuse. While Screening is a budget-friendly approach to dismiss drug use for most specimens, a positive screen requires confirmation for legal purposes. Any samples preliminarily positive during Screening necessitate further confirmation.

If a specimen tests presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), thus hair drug screening is among the most challenging methodologies to perform.

A positive screening result's Confirmation is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The full laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
