What it is, How it works

When sourced near the scalp, hair offers roughly a 3-month timeframe for detecting alcohol and other substances. Hair collection is straightforward, relatively tamper-resistant, and convenient for shipping.

A 1.5-inch section of around 200 hair strands (about the thickness of a #2 pencil) near the scalp will yield 100mg of hair, the optimal quantity for screening and verification. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's recommended to measure the sample using a jeweler’s scale. If scalp hair isn't accessible, a comparable amount of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in a lab drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with entering a sample into a lab's system. Tasks include checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and inputting any extra data not covered by an electronic custody system.

Screening involves a preliminary rapid assessment for drug traces. Though Screening is an economical means to filter out drug use in most samples, a positive result requires confirmation for court validity. Samples showing potential positives in Screening necessitate further confirmation.

If Screening suggests a positive, additional hair is extracted from the original sample for Preparation. During this phase, substances are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), explaining why hair drug screening is among the most challenging techniques.

Confirmation of any positive screen results is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All positive indications are cleaned before confirmation as needed. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
