What it is, How it works

Hair offers an extended history of alcohol and drug intake by capturing biomarkers within the strands of growing hair. When collected near the scalp, hair testing can reveal up to a roughly three-month period of alcohol and drug detection. It's straightforward to gather, relatively tough to tamper with, and simple to transport.

A sample measuring 1.5 inches with around 200 hair strands (comparable to the diameter of a #2 pencil) closest to the scalp provides 100mg of hair, considered the optimal amount for screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg of hair is advised. We suggest utilizing a jeweler’s scale to weigh the sample. If scalp hair isn't accessible, a comparable quantity of body hair can be used. We refer to head hair as only scalp hair. Body hair pertains to all other types, like facial, axillary, etc.

Process Overview

The lab processing involved in drug test analysis consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample into a laboratory’s framework. This step ensures the sample is correctly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and incorporates any extra data entry not supplied by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug abuse presence. Though Screening is a budget-friendly approach to exclude drug use in most samples, a positive result must be validated for legal acceptance. Any samples that are presumed positive in Screening do necessitate a secondary verification.

If the Screening step indicates a presumptive positive result, additional hair is taken from the original specimen and readied for Extraction. Drugs are extracted from hair with significantly lower concentration compared to other methods (like urine or oral fluid), highlighting why hair drug screening is highly complex.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are cleansed before confirmation as necessary. The full laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
