A sample measuring 1.5 inches and comprising around 200 strands (akin to the thickness of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample for analysis and confirmation. For EtG, additional tests, and/or assessments exceeding 10 panels, a 150mg sample is advisable. We suggest using a jeweler’s scale for weighing the sample. If scalp hair cannot be sourced, an equivalent amount of body hair is an alternative. When mentioning head hair, we refer solely to scalp hair. Body hair denotes any other hair types, such as facial, axillary, etc.

Process Overview

The lab treatment of a drug test result engages four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the fundamental processing of a sample within a lab’s infrastructure. This step includes verifying appropriate sealing and shipping of the sample, assigning it a non-specific LAN (Laboratory Accessioning Number), and finalizing any supplementary data input absent from an electronic chain of custody system.

Screening is a preliminary and swift assessment for signs of drug abuse. Although Screening offers an economical means to rule out drug use in most samples, a positive screen must undergo confirmation to hold up in court. Samples showing presumptive positives in Screening necessitate a subsequent confirmation test.

If a sample indicates a presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction phase. Here, drugs are withdrawn from hair at a notably lower concentration compared to alternative methods (such as urine or oral fluid), which is why hair drug screening represents the most intricate methodology.

Any positive screening results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All presumptive positive samples undergo washing prior to confirmation if necessary. The full lab cycle from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
