In scenarios where the Screening stage indicates a presumptive positive, additional hair is extracted from the original specimen and prepped for Extraction. At this phase, drugs are extracted from hair in significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which explains why hair drug screening is a complex methodology to execute.

The Confirmation of any presumptive positive screening result is processed using GC/MS, GC/MS/MS, or LC/MS/MS. Necessary washing of all presumptive positive samples is conducted prior to confirmation. The entire lab process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation as well as accredited against the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
