What it is, How it works

When gathered close to the head, hair can offer a detection period of up to nearly 90 days for alcohol and drugs. Hair is straightforward to collect, relatively resistant to tampering, and simple to transport.

A sample consisting of 1.5 inches of approximately 200 hairs (roughly akin to a #2 pencil size) nearest to the scalp yields 100mg of hair, which is the optimal sample for both screening and verification. For EtG, supplementary tests, and/or panels exceeding 10, a collection of 150mg is advised. Using a jeweler’s scale is suggested for weighing the specimen. If hair from the scalp is unavailable, equivalent body hair may be utilized. References to head hair specifically denote scalp hair. Body hair comprises other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial input of a sample into the lab system, ensuring the sample was properly sealed and transported, assigning a Laboratory Accessioning Number (LAN) randomly, and completing any extra data not covered by an electronic chain of custody.

Screening acts as an initial swift examination for substance abuse. While it is a cost-efficient way to dismiss drug use for most samples, a positive indication requires confirmation to be legally valid. Any samples flagged as potentially positive in Screening necessitate further verification.

Should a sample indicate a positive result in the Screening phase, additional hair is extracted from the original sample and readied for Extraction. Here, drugs are isolated from hair at concentrations much lower than other methods (such as urine or oral fluid), which makes hair analysis the most challenging method to carry out.

Positive results in Screening are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive specimens undergo washing before confirmation if necessary. Each step from Accessioning to Confirmation is audited against both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
