Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing within the laboratory's system. This stage includes confirming the sample's seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and inputting any additional data not captured by an electronic chain of custody system.

The Screening phase consists of a preliminary assessment for substances of abuse. While Screening is an economical approach for eliminating drug use in most samples, a positive screen must be confirmed to be valid in legal contexts. Any samples showing a presumptive positive in Screening necessitate further confirmation.

Should the sample indicate a presumptive positive in Screening, additional hair is taken from the initial sample for the Extraction stage. At this point, drugs are extracted from the hair at a significantly lower concentration than found in other methods (such as urine or oral fluid), underscoring the complexity of hair drug screening.

GC/MS, GC/MS/MS, or LC/MS/MS techniques are employed for confirming any positive screening outcome. All samples that indicate a presumptive positive are cleansed before confirmation as required. The whole laboratory process, from Accessioning to Confirmation, is subjected to scrutiny under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
