What it is, How it works

When taken from near the scalp, hair can present a detection window of up to roughly 3 months for alcohol and drugs. Hair collection is straightforward, hard to tamper with, and convenient to transport.

A 1.5-inch segment consisting of approximately 200 hair strands (roughly the size of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, supplementary analyses, or tests exceeding 10 panels, a specimen of 150mg is advisable. It is recommended that this specimen be weighed using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair may be utilized. When mentioning head hair, we are specifically referring to scalp hair only. Body hair includes all other types (facial, underarm, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of integrating a sample into a laboratory’s system initially. This phase includes verifying the appropriate sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not included by an electronic chain of custody system.

Screening is an initial swift assessment for drugs of abuse. While Screening offers a cost-efficient method to exclude drug usage in most samples, any positive screen requires confirmation to be considered valid in judicial settings. Samples that give a presumptive positive in Screening must undergo a secondary confirmation.

If a Screening stage indicates a presumptive positive, additional hair is taken from the initial sample for preparation in the Extraction. In this stage, drugs are extracted from hair at significantly lower concentrations than in other methodologies (for instance, urine or oral fluids), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before necessary confirmation. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
