An ideal sample size for analysis and confirmation is a segment of 1.5 inches comprising about 200 hair strands (roughly the thickness of a #2 pencil), which provides 100mg of hair. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is suggested. It is advised to use a jeweler’s scale for weighing the specimen. If scalp hair is inadequate, a proportionate amount of body hair can be used. When mentioning head hair, it specifically refers to scalp hair. Conversely, body hair refers to all other types (e.g., facial, axillary).

Process Overview

The laboratory processing for drug test outcomes encompasses four critical steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the lab’s system, which involves verifying the sample's seal and shipment, generating a unique LAN (Laboratory Accessioning Number), and updating any additional data not covered by an e-chain of custody.

Screening signifies an initial rapid assessment for substances that are abused. While Screening efficiently excludes drug use for most samples, a confirmed result is necessary for court admissibility. Samples showing a presumptive positive in Screening necessitate further confirmation.

If a sample appears to be positive in the Screening phase, additional hair is extracted from the original sample for the Extraction process. In this phase, substances are withdrawn from hair at much lower concentrations than other techniques (e.g., urine, oral fluids), which renders hair drug testing the most challenging technique to execute.

Confirmation of any presumptive positive results involves the use of GC/MS, GC/MS/MS, or LC/MS/MS. All samples hinting at positivity are cleansed prior to confirmation if required. The entire laboratory sequence from Accessioning to Confirmation is examined following the CAP (College of American Pathologists) Hair specifications as well as ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
