A sample of 1.5 inches, consisting of about 200 hair strands (around the thickness of a #2 pencil) nearest to the scalp, will provide 100mg, which is the ideal size for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, it is recommended to use 150mg of the sample. We suggest using a jeweler’s scale for the specimen’s weight. If hair from the scalp is not accessible, a comparable quantity of body hair can be used. When mentioning head hair, it strictly pertains to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary handling of a sample within the laboratory’s system. This step includes verifying that the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening entails an initial swift check for drug abuse. While Screening is a cost-effective measure to eliminate drug use from most samples, a positive screen necessitates confirmation to be acceptable in court. Any samples showing presumptive positive results in Screening require a subsequent confirmation.

If a sample shows presumptive positive results during Screening, additional hair is pulled from the original specimen and prepared for Extraction. At this stage, drugs are extracted from hair in much lower amounts than through other means (e.g., urine or oral fluids), making hair drug screening one of the most complex methodologies to perform.

Confirmation of any positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
