For EtG, extra tests, and/or panels exceeding 10, 150mg is advised. We suggest measuring the specimen on a jeweler's balance. If scalp hair isn't present, an equivalent portion of body hair can be taken. "Head hair" refers specifically to scalp hair, whereas "body hair" encompasses all others (such as facial or axillary hair).

Process Overview

The process entailing four primary phases in lab analysis of drug test results includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails introducing a sample into the lab’s framework. This step verifies proper sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any remaining data tasks not fulfilled by an electronic chain of custody.

Screening gives a preliminary examination for substance use. While cost-effective for clearing most samples of drug use, any positive outcome during Screening demands additional verification for court validity. Presumptively positive samples necessitate a follow-up confirmation.

When Screening suggests a positive result, more hair is derived from the initial specimen for Extraction. Drugs are retrieved from hair at a reduced concentration compared to alternative techniques (e.g., urine or saliva), highlighting the challenges in hair drug analysis.

Confirming a positive screen involves methodologies like GC/MS, GC/MS/MS, or LC/MS/MS. All samples flagged as presumptive positives are cleansed before confirmation as necessary. The entire process, from Accessioning to Confirmation, undergoes scrutiny under the CAP (College of American Pathologists) Hair designation alongside ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
