Cutting a 1.5-inch segment of approximately 200 hair strands, comparable in thickness to a #2 pencil and nearest to the scalp, will yield 100mg of hair, which is optimal for both screening and verification. For EtG, supplements, and/or tests surpassing 10 panels, a specimen of 150mg is advisable. To ensure accuracy, weigh the sample on a jeweler’s scale. In cases where scalp hair isn't available, an equivalent amount of body hair can be obtained. References to head hair imply scalp hair exclusively, whereas body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The quartet of primary stages in the laboratory processing of a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning details the initial intake of a sample into the lab system. This step involves verifying the proper sealing and shipment of the sample, assigning it a random LAN (Laboratory Accessioning Number), and addressing any supplementary data entry that isn't covered by an electronic chain of custody system.

Screening consists of an initial, rapid assessment for substance abuse. Although cost-efficient for negating drug use in most samples, a positive screening must be confirmed for legal acceptability. Any samples that exhibit a presumptive positive during Screening require subsequent affirmation.

Should a sample indicate a presumptive positive during Screening, additional hair is retrieved from the starting sample and readied for Extraction. At this juncture, substances are drawn from hair at notably lower concentrations than with alternate methods (e.g., urine or oral fluids), rendering hair drug analysis a challenging process to execute.

The Confirmation of any positive Screening result is executed utilizing GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to Confirmation, all presumptive positive samples undergo washing as necessary. The end-to-end laboratory workflow from Accessioning to Confirmation is overseen in accordance with the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
