For EtG, additional testing, and/or tests exceeding ten panels, 150mg of the specimen is suggested. It's advisable to weigh the specimen using a jeweler's scale. If scalp hair isn't accessible, an equal portion of body hair can be utilized. By head hair, we refer exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug tests encompasses four principal steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample enters a lab’s system, entailing the verification of sealing and shipping integrity, the assignment of a LAN (Laboratory Accessioning Number), and the completion of any supplementary data entry absent from an electronic chain of custody system.

Screening conducts an initial quick assessment for substances of abuse. While Screening provides a cost-efficient method to rule out drug use for most samples, any positive screen must be corroborated for legal admissibility. Samples preliminarily positive in Screening necessitate confirmatory testing.

If Screening indicates a presumptive positive, additional hair is taken from the initial sample for Extraction. In this phase, drugs are extracted from hair at concentrations lower than other methods (e.g., urine or oral fluid), making hair drug screening notably complex.

Positive screenings are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positives undergo washing before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation is assessed under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
