What it is, How it works

A 1.5-inch segment comprising approximately 200 hair strands (around the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is the perfect specimen for both screening and confirmation. For EtG, additional tests, and/or assessments beyond 10 panels, a 150mg sample is advisable. It is suggested to measure the sample using a jeweler’s scale. In cases where scalp hair is unavailable, an equivalent quantity of body hair can be collected. By head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (e.g., facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample into a laboratory's system. This process includes ensuring the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening involves a preliminary rapid check for substances of abuse. While Screening is an economical approach to exclude drug usage in most samples, a positive result requires confirmation to be valid for legal purposes. Any samples with presumptive positive results in Screening need a secondary confirmation.

If a sample shows presumptive positivity in the Screening phase, additional hair is taken from the original sample for Extraction. In this step, drugs are extracted from hair at substantially lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive Screening findings is carried out via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The complete lab processing from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair specification and the ISO / IEC 17025 standard accreditation.
