A 1.5-inch segment consisting of around 200 hair strands (roughly the dimension of a #2 pencil) from close to the scalp yields 100mg of hair, ideal for screening and confirmation. For EtG, additional substances, and/or tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to measure the sample is recommended. If scalp hair isn’t available, a similar quantity of body hair may be used. The term "head hair" refers specifically to scalp hair, whereas "body hair" covers all other hair types (such as facial and axillary).

Process Overview

The procedure of processing a drug test result in a laboratory involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

The initial phase, Accessioning, incorporates the sample into the laboratory's system. This step includes verifying the seal and proper shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data not captured by an electronic chain of custody system.

Screening consists of an initial rapid assessment for drug abuse. Although Screening provides a cost-efficient method to eliminate drug use in most samples, it necessitates confirmation for a positive result to be considered valid in legal contexts. Samples testing positive in Screening must be confirmed through a secondary method.

If a sample is positive in the Screening phase, additional hair is extracted from the initial sample for the Extraction phase. In this phase, drugs are removed from hair at significantly lower concentrations compared to other methods like urine or oral fluid, which makes hair drug screening the most challenging technique to execute.

Confirmation of a positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptively positive samples are cleansed as required before confirmation. The complete laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair category and the accreditation of ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
