What it is, How it works

Hair illustrates a prolonged history of alcohol and other drugs by embedding biomarkers in the fibers of the developing hair strand. When gathered near the scalp, hair can offer up to a 3-month detection period for alcohol and other drugs. Hair is easy to gather, somewhat resistant to tampering, and simple to transport.

A 1.5-inch sample of roughly 200 hair strands (about the width of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the perfect sample for screening and confirmation. For EtG, additional screenings, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair isn't available, a comparable amount of body hair might be collected. When mentioning head hair, we are solely referring to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The principal steps involved in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a laboratory’s database. This process involves validating the proper sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any extra data input not furnished by an electronic chain of custody system.

Screening consists of an initial swift evaluation for drugs of abuse. While Screening is an economical method to dismiss drug usage for most samples, a positive screen requires confirmation to be valid in legal settings. Any samples that are presumptively positive in Screening must undergo a secondary confirmation.

If a sample is deemed presumptively positive during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than in other techniques (e.g., urine or oral fluid), which is why hair drug screening is the most challenging technique to perform.

Confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
