Hair follicle tests are chosen by those with strict drug policies, legal entities, and probation cases. The extended detection period, up to 90 days, collected through observation, makes them nearly foolproof, except when testing recent usage within the past 5 days, where urine tests prevail.

What it is, How it works

Hair analysis is increasingly acknowledged as an effective method for identifying drug and alcohol consumption. Hair offers an extended historical record of alcohol and drug use by trapping biomarkers within the growing strands. Collecting hair adjacent to the scalp can yield a detection period of up to three months for alcohol and other substances. Hair collection is straightforward, challenging to tamper with, and conveniently shippable.

A 1.5-inch segment comprising approximately 200 strands (similar in size to a #2 pencil) nearest the scalp will produce 100mg of hair, which is the optimal sample size for screening and verification. For EtG, supplementary, and/or tests exceeding 10 panels, 150mg of the sample is suggested. Using a jeweler’s scale to weigh the specimen is advisable. If scalp hair is not accessible, an equivalent quantity of body hair is acceptable. When referencing head hair, it pertains solely to scalp hair. Body hair refers to any other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing drug test outcomes encompasses four critical steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's framework. This process includes verifying the sample's proper sealing and shipment, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening involves an initial rapid assessment for substances of abuse. Although Screening is a cost-efficient means of excluding drug use for most samples, a positive result necessitates confirmation to be court-admissible. Any samples that test presumptively positive during Screening require an additional confirmation.

Should a sample be presumptively positive during Screening, additional hair is extracted from the initial specimen and readied for Extraction. At this phase, drugs are isolated from hair at significantly lower concentrations than in alternative methods (e.g., urine or oral fluid), making hair drug analysis the most challenging technique to execute.

Confirmation of any positive result from Screening is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation when necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation compliant with ISO / IEC 17025 standards.

Note: Although often described as "hair follicle tests", the analysis is conducted on the hair strand itself, not the follicle beneath the scalp.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
