What it is, How it works

Hair analysis has become recognized as a potent method for detecting drug and alcohol consumption. Hair offers an extended record of alcohol and other substances by capturing biomarkers within the strands' fibers. When taken near the scalp, it can reveal up to a roughly 3-month period for detecting alcohol and other drugs. Collecting hair is straightforward, relatively hard to alter, and convenient to transport.

A 1.5-inch portion of roughly 200 hair strands (about the width of a #2 pencil) nearest the scalp yields 100mg of hair, which is the preferred sample size for both screening and confirmation. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair is unavailable, a comparable quantity of body hair can be obtained. When mentioning head hair, it refers specifically to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary phases in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample into a lab's system. This process includes confirming that the sample was adequately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any necessary data input not covered by an electronic chain of custody system.

Screening entails a preliminary swift evaluation for substances of abuse. Although Screening is an economical means to exclude drug use in most samples, a positive outcome must be verified for court acceptance. Any samples that are tentatively positive in Screening require further verification.

If a sample shows a presumptive positive result in the Screening phase, supplementary hair is taken from the original sample and readied for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most intricate methodology.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are washed as deemed necessary before confirmation. The overall lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Prolonged detection period: Hair drug tests can identify drug usage for up to 90 days, unlike urine tests which have a shorter detection period.
  • Difficult to manipulate: It is notably challenging to manipulate a hair drug test, rendering the outcomes more reliable.
  • Offers historical insight: It can display a pattern of substance use over time, not merely recent activity.

Limitations:

  • Inability to detect recent use: It typically takes 5-7 days for substances to become detectable in hair.
  • Expense: Hair drug tests are usually pricier than other drug testing options.
  • Result variability: Elements such as hair color and individual differences in hair growth can influence the concentration of drug metabolites within hair.

Note: Although often termed "hair follicle tests", the analysis targets the hair strand itself and not the follicle under the scalp.
