When taken near the scalp, it can reveal up to a roughly 3-month period for detecting alcohol and other drugs. Collecting hair is straightforward, relatively hard to alter, and convenient to transport.

A 1.5-inch portion of roughly 200 hair strands (about the width of a #2 pencil) nearest the scalp yields 100mg of hair, which is the preferred sample size for both screening and confirmation. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair is unavailable, a comparable quantity of body hair can be obtained. When mentioning head hair, it refers specifically to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary phases in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample into a lab's system. This process includes confirming that the sample was adequately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any necessary data input not covered by an electronic chain of custody system.

Screening entails a preliminary swift evaluation for substances of abuse. Although Screening is an economical means to exclude drug use in most samples, a positive outcome must be verified for court acceptance. Any samples that are tentatively positive in Screening require further verification.

If a sample shows a presumptive positive result in the Screening phase, supplementary hair is taken from the original sample and readied for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most intricate methodology.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are washed as deemed necessary before confirmation. The overall lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
