They are collected under supervision and are nearly immune to tampering. However, for detecting drug use within the recent five days, urine tests remain the most precise.

What it is, How it works

Hair testing is now widely recognized as a potent method for identifying drug and alcohol consumption. Hair encapsulates a long-term record of substances by embedding biomarkers within the fibers of the hair as it grows. When taken from near the scalp, hair can indicate drug and alcohol use for up to around 3 months. Hair is straightforward to collect, somewhat challenging to tamper with, and convenient to transport.

Collecting a sample of 1.5 inches of roughly 200 strands of hair (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for both screening and confirmation. For tests such as EtG, add-ons, and/or those beyond 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler's scale. If scalp hair isn't available, an equivalent quantity of body hair can be used. When we talk about head hair, we're specifically indicating scalp hair. Body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The primary stages in the lab analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves entering the sample initially into a lab's system. This process includes ensuring the sample was properly sealed and sent, assigning it a random LAN (Laboratory Accessioning Number), and completing any extra data entry that is not managed by an electronic chain of custody system.

Screening is an initial rapid examination for drugs of abuse. While Screening is a cost-efficient approach to eliminate drug usage in most samples, a positive result must undergo confirmation to be legally admissible. Samples that are presumptively positive in Screening require an additional confirmation step.

If a sample tests as presumptively positive in the Screening stage, more hair is taken from the initial sample and prepped for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug testing is the most challenging method to perform.

Confirmation of any positive screening outcome is done using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The full lab processing from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
