What it is, How it works

Hair sampling has emerged as an effective means for identifying drug and alcohol consumption. Hair holds a continuous record of drug and alcohol use by embedding biomarkers in the fibers of the growing hairs. When taken close to the scalp, hair can indicate up to a 3-month history of drug and alcohol use. It's easy to gather, somewhat resistant to tampering, and convenient for transport.

A lock of hair measuring 1.5 inches with around 200 hairs (about the size of a #2 pencil) nearest to the scalp provides around 100mg, which is optimal for analysis and verification. For EtG, extended panels, or examinations beyond 10 panels, 150mg of hair is suggested. We advise measuring the hair on a precise jeweler’s scale. If head hair isn't available, a comparable amount of body hair can be used. By head hair, we mean only scalp hair, whereas body hair includes all other types (facial, underarm, etc.).

Overview of the Procedure

The drug test outcome goes through four major phases in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the first processing phase of the sample in a laboratory's workflow. This involves validating that the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and filling in any additional data not captured by an electronic tracking system.

Screening entails an initial swift assessment for drug use. Although Screening is cost-efficient to exclude drug usage for most samples, a positive result needs confirmation to be considered valid in legal proceedings. Any samples identified as positive in Screening necessitate a further confirmation.

Should a sample show positive signs during Screening, additional hair is extracted from the original sample for the Extraction phase. During this phase, drugs are removed from hair in significantly lower concentrations than in other practices (e.g., urine or saliva), making hair drug testing the most complex technique to execute.

Confirming any positive outcomes from screening is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing preliminary positive are cleansed before the confirmation as needed. The entire laboratory workflow from Accessioning through to Confirmation complies with both CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation criteria.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
