What it is, How it works

Collected near the scalp, hair offers roughly a 3-month detection period for alcohol and drug presence. Hair is easy to gather, relatively hard to tamper with, and straightforward to send.

A 1.5-inch piece containing around 200 hair strands (comparable to a #2 pencil in width) nearest the scalp provides 100mg, the optimal amount for screening and verification. For EtG, enhancements, and/or tests exceeding 10 panels, 150mg is preferred. We advise measuring the sample with a jeweler's scale. If scalp hair is unavailable, a matching quantity of body hair can be used. Head hair refers solely to scalp hair, whereas body hair covers other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a specimen in a lab's system. This step ensures the sample is sealed and delivered properly, assigns an arbitrary LAN (Laboratory Accessioning Number), and inputs any extra data not offered by an electronic chain of custody.

Screening consists of an initial rapid examination for substance misuse. While cost-effective, a positive result in Screening necessitates confirmation for legal admissibility. Presumptively positive samples in Screening require follow-up confirmation.

If Screening indicates a presumptive positive, more hair is extracted and prepped for Extraction. At this stage, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of any screening positives is achieved through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positives undergo washing before confirmation if required. The entire lab procedure, from Accessioning to Confirmation, is examined under CAP (College of American Pathologists) Hair certification and compliance with ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
