In recent times, hair follicle testing has increasingly become prevalent in drug testing methodologies. More businesses, judicial systems, and substance abuse specialists are opting for hair drug tests over traditional urine analysis. Hair follicle tests are favored by organizations with strict drug-free policies, legal authorities, and persons under probation because they offer an extended drug-use detection period of up to 90 days and are incredibly difficult to falsify. However, when immediate past drug use is suspected, within the last five days, urine analysis remains unmatched in precision.

What it is, How it works

Hair testing has emerged as a significant method for identifying drug and alcohol consumption. Hair retains a prolonged record of substance use by embedding biomarkers within the strands of the growing hair. By collecting hair near the scalp, it can reveal up to a three-month period of drug and alcohol detection. Hair sample collection is straightforward, hard to tamper with, and convenient to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil) near the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, or tests over 10 panels, a sample of 150mg is advisable. It is suggested to measure the specimen using a jeweler's scale. If there is no scalp hair, a similar quantity of body hair can be used. By head hair, we mean only scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

There are four primary steps in the lab analysis of a drug test: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the lab’s system, including checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not done by an electronic chain of custody system.

Screening involves a preliminary examination for drugs of abuse. While Screening is an economical way to exclude drug use in most samples, any positive results must be confirmed to be valid in a legal setting. Any samples that yield a presumptive positive result in Screening should undergo a secondary confirmation.

If a sample is found presumptive positive at the Screening stage, more hair is extracted from the original sample for the Extraction process. In this phase, drugs are removed from hair at concentrations much lower than other methods (e.g., urine or saliva), which makes hair drug screening one of the most challenging methodologies to execute.

Confirmation for any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The whole laboratory process from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair designation and the endorsement to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
