To perform EtG add-ons or tests exceeding 10 panels, a specimen weighing 150mg is advisable. Utilizing a jeweler's scale to measure the specimen is suggested. In the absence of scalp hair, a comparable quantity of body hair can be obtained. "Head hair" specifically denotes scalp hair, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result involves four core steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the laboratory's system, ensuring that it was properly sealed and shipped. It includes assigning a unique LAN (Laboratory Accessioning Number) and addressing any additional data entry that is not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for potential drug abuse, serving as a cost-effective means of eliminating drug use for a large portion of samples. However, a positive screening requires confirmation to be considered valid in legal proceedings. Samples with a presumptive positive result in screening necessitate further confirmation.

If a screening indicates a presumptive positive, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted at much lower concentrations compared to other testing methods like urine or oral fluid, highlighting why hair drug testing represents a more complex methodology.

Confirmation of any positive results from screening is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive undergo washing prior to confirmation if necessary. The complete laboratory procedure from Accessioning through Confirmation is subject to review in accordance with both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
