For EtG tests and screenings exceeding 10 panels, a 150mg sample is advised. It's recommended to use a jeweler’s scale for weighing the sample. Should scalp hair be unavailable, an equivalent quantity of body hair may be gathered. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The lab processing for drug test outcomes involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a lab's workflow. This step ensures that the sample was properly sealed and delivered, allocates a random LAN (Laboratory Accessioning Number), and completes any extra data input not captured by an electronic chain of custody system.

Screening constitutes a preliminary assessment to detect drug presence. Though it is an economical approach for eliminating drug use in most samples, any positive screen result necessitates confirmation for legal acceptability. Samples showing presumptive positives in Screening need a secondary confirmation.

When a sample shows a presumptive positive result during Screening, additional hair is taken from the original sample for the Extraction process. During this phase, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluids), which makes hair drug screening particularly challenging.

Final confirmation of any positive screen result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples testing presumptive positive are washed prior to confirmation where necessary. From Accessioning to Confirmation, the entire lab process is scrutinized following both the CAP (College of American Pathologists) Hair certification and the ISO / IEC 17025 standard accreditations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
