When gathered near the scalp, hair offers a detection range for alcohol and drugs, extending up to about 3 months. Hair collection is straightforward, relatively hard to tamper with, and convenient for transportation.

A 1.5-inch segment consisting of approximately 200 hair strands (comparable in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for screening and verification. For assessments involving EtG, additional substances, or those exceeding 10 panels, a 150mg sample is advisable. We suggest using a jeweler’s scale to weigh the sample. If scalp hair can't be obtained, a similar amount of body hair may be used. When we mention head hair, we specifically mean scalp hair. Body hair denotes all other hair categories (such as facial, axillary, etc.).

Process Overview

The laboratory method for processing a drug test result primarily includes four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a lab’s framework. It includes confirming the sample’s proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid inspection for substance abuse. Although Screening is a budget-friendly way to dismiss drug use in most cases, a positive result requires confirmation to hold up in legal proceedings. Any samples that are presumptively positive in Screening require an additional verification.

For samples with presumptive positive results in Screening, more hair is extracted from the initial sample and arranged for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Verification of any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive specimens are cleansed prior to verification as necessary. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
