What it is, How it works

Hair testing is now regarded as an effective method for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and drugs by locking biomarkers in the fibers of the growing hair strand. When taken close to the scalp, hair can offer up to roughly a 3-month detection period for alcohol and drugs. Hair is easy to collect, somewhat hard to tamper with, and convenient to ship.

A 1.5-inch piece of about 200 hair strands (around the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, the optimal sample for testing and confirmation. For EtG, additional tests, and/or tests over 10 panels, 150mg of the specimen is suggested. We advise weighing the specimen on a jeweler’s scale. If scalp hair is lacking, an equivalent amount of body hair can be taken. When mentioning head hair, we refer specifically to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial introduction of a sample into a lab’s system. This entails ensuring that the sample was sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any extra data entry not covered by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Though Screening is a cost-effective method to rule out drug usage in most samples, a positive screen demands confirmation for court admissibility. Samples that are presumed positive in Screening require an additional confirmation.

If Screening suggests a sample is positive, more hair is taken from the original specimen and prepared for Extraction. During this stage, drugs are removed from hair at a much lower concentration than other methods (ex. urine or oral fluid), which is why hair drug screening is the most complex methodology to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
