What it is, How it works

A sample procured near the scalp can reveal a substance history extending roughly up to three months. Hair is easy to gather, challenging to tamper with, and straightforward to dispatch.

Collecting a 1.5-inch segment, consisting of about 200 hair strands (approximately the girth of a #2 pencil), yields 100mg, the optimal sample for screening and confirmation. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. The specimen's weight should be assessed using a jeweler’s scale. If scalp hair is inaccessible, an equivalent quantity of body hair is acceptable. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four fundamental steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes its initial processing into the laboratory system. This step includes verifying proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry that an electronic chain of custody system might not cover.

Screening serves as a preliminary rapid assessment for drug presence. Although cost-effective for eliminating suspected drug use in most samples, a positive screen necessitates further validation to hold up in legal proceedings. Samples yielding positive presumptions in Screening mandate additional confirmation.

Should Screening suggest a positive outcome, further hair extraction from the original sample commences. During Extraction, drugs are extracted at considerably lower concentrations compared to other methods (e.g., urine or oral fluid), marking hair drug screening as a notably challenging methodology.

Confirmation of any positive screening findings utilizes techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, wash procedures are applied to all suspected positive samples. Each lab phase from Accessioning to Confirmation is scrutinized under the oversight of the CAP (College of American Pathologists) Hair guidelines and aligned with the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
