What it is, How it works

When collected near the scalp, hair can offer an approximately 3-month detection period for alcohol and drug presence. Collecting hair is straightforward, relatively hard to tamper with, and easy to transport.

A sample measuring 1.5 inches consisting of about 200 hair strands (roughly the dimensions of a #2 pencil) closest to the scalp will provide about 100mg, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or analyses beyond 10 panels, a 150mg specimen is recommended. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent amount of body hair can be used. In the context of head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into a lab's system. This stage encompasses validating that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening consists of an initial rapid examination for drugs of abuse. While Screening is a cost-efficient means of excluding drug use in most samples, a positive screen needs validation to be valid in court. Any samples that test positive in Screening require a secondary confirmation.

If a sample tests positive in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than with other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to carry out.

Confirmation of any positive screening outcomes is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed as necessary before confirmation. The entire laboratory process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
