These tests are collection-process observed, reducing the chance of manipulation significantly. However, urine tests still lead in accuracy for detecting recent drug use within the past 5 days.

What it is, How it works

Hair testing is recognized as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of substances by embedding biomarkers within the strands of hair as it grows. When hair is collected near the scalp, it can reveal a detection period of up to roughly 3 months for drugs and alcohol. It is easy to collect hair, challenging to tamper with, and simple to mail.

An ideal sample for analysis and verification consists of a 1.5-inch section comprising around 200 hair strands (similar in size to a #2 pencil) closest to the scalp, totaling 100mg. For EtG, add-ons, and/or tests covering more than 10 panels, 150mg of the sample is advised. We suggest using a jeweler's scale to measure the sample weight. If no scalp hair is available, a comparable quantity of body hair may be used. By head hair, we mean scalp hair exclusively. Other hair refers to all non-scalp hair (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial intake into the lab system. This stage includes confirming the sample's proper sealing and shipping, generating a random LAN (Laboratory Accessioning Number), and completing any missing data entry not available through an electronic chain of custody.

Screening involves a preliminary rapid assessment for illegal substances. Screening offers a cost-efficient means to eliminate suspicion of drug use in most samples; however, any positive screen findings need confirmation for legal validity. Samples testing preliminarily positive in Screening require subsequent confirmation.

For samples preliminarily positive at the Screening level, more hair is extracted from the original sample and readied for Extraction. During this stage, drugs are pulled from hair at significantly lower concentrations than via other methods (e.g., urine or oral fluid), making hair drug screening the most advanced method to undertake.

Confirmation of any results from Screening occurs using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test preliminary positive are cleansed before confirmation as necessary. The entire laboratory operation from Accessioning through to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
