A 1.5-inch segment of approximately 200 hair strands (roughly equivalent to the size of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample for both screening and verification. For EtG, additional testing, and/or panels exceeding 10, a 150mg specimen is advised. Utilizing a jeweler’s scale to weigh the sample is suggested. If scalp hair is not available, an equal quantity of body hair may be used. When discussing head hair, we denote only scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing within a laboratory’s framework, including verification of proper sample sealing and shipping, LAN (Laboratory Accessioning Number) assignment, and completion of any supplementary data entry absent from an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs of abuse. While Screening efficiently rules out drug usage for most samples cost-effectively, a positive screen must undergo confirmation to be court-admissible. Presumptively positive Screening samples require secondary validation.

Should a sample show presumptive positivity during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. During this phase, drug extraction from hair occurs at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), which explains the complex nature of hair drug screening.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive undergo necessary pre-confirmation washing. The entire laboratory process, from Accessioning to Confirmation, is conducted under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
