What it is, How it works

When obtained near the scalp, hair can offer approximately a 3-month detection period for alcohol and other substances. Hair collection is straightforward, somewhat challenging to tamper with, and simple to transport.

A 1.5-inch sample comprising roughly 200 hair strands (about the thickness of a #2 pencil) nearest the scalp yields 100mg of hair, the ideal quantity for screening and validation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the specimen is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not present, an equivalent amount of body hair can be used. When referencing head hair, we mean scalp hair only. Body hair denotes all other types of hair (facial, axillary, etc.).

Process Overview

There are four main stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial input of a sample into a lab's system. This involves confirming the sample was correctly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not available from an electronic chain of custody system.

Screening consists of an initial swift examination for drugs of concern. While Screening is an economical way to eliminate drug use in most samples, a positive screen must be confirmed for legal acceptability. Any samples that are presumptively positive in Screening need a secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is taken from the original specimen and prepared for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex method to execute.

Confirmation of any positive screening result is completed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are washed before confirmation if necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
