For EtG, add-ons, or tests exceeding 10 panels, 150mg of the specimen is advisable. We recommend utilizing a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent quantity of body hair may be obtained. When mentioning head hair, we refer strictly to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s system. This step involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening performs an initial rapid checking for drugs of abuse. While Screening serves as a cost-efficient method to exclude drug use in the majority of samples, a positive result requires confirmation to be considered valid in court. Any samples presumptively positive during Screening necessitate subsequent confirmation.

Should a sample be presumptively positive in Screening, additional hair is drawn from the initial specimen and readied for Extraction. Drugs are extracted from hair at significantly lower concentrations than in alternative methodologies (e.g., urine or oral fluid), making hair drug screening particularly challenging to perform.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation if needed. The entire laboratory process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
