For EtG, add-ons, or tests exceeding 10 panels, a specimen of 150mg is suggested. Using a jeweler’s scale for weighing the sample is advised. In the absence of scalp hair, a comparable amount of body hair can be collected. "Head hair" refers solely to scalp hair, while "body hair" encompasses all other types, such as facial and axillary hair.

Process Overview

The primary phases in processing a drug test result at the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is entered into the laboratory system. This step involves checking that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entries not provided through an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening efficiently rules out drug usage for most samples cost-effectively, a positive result necessitates confirmation for legal admissibility. Samples that test presumptively positive in Screening require a secondary confirmation.

Upon a presumptive positive in Screening, additional hair is extracted from the primary specimen for the Extraction phase. Here, drugs are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug analysis particularly challenging.

Positive screening confirmations are conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive results undergo washing as required before confirmation. The entirety of the laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair standards and the accreditation to ISO / IEC 17025 standards.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
