Hair collection is straightforward, relatively tamper-proof, and easy to transport.

A 1.5-inch portion, consisting of roughly 200 hair strands (comparable to the thickness of a #2 pencil) nearest the scalp, provides 100mg of hair, ideal for both screening and confirmation. For EtG, additional tests, or panels exceeding 10, 150mg is advisable. Using a jeweler's scale to measure the specimen is suggested. If scalp hair isn’t available, an equivalent amount of body hair can be used. When discussing head hair, it specifically refers to scalp hair, whereas body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory steps for processing drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning represents the initial sample processing into a lab's system. It involves confirming the sample was appropriately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry that wasn't included by an electronic chain of custody system.

Screening is a preliminary examination for drugs of abuse. Although Screening is economical for ruling out drug use in most samples, any positive findings need confirmation to be valid legally. Samples that show presumptive positives in Screening must undergo additional confirmation.

If a sample appears positive during Screening, further hair is taken from the original sample and prepped for Extraction. At this point, substances are extracted from hair at notably lower concentrations than with other techniques (e.g., urine or oral fluid), making hair drug testing the most challenging methodology.

Confirmed positive screening results are verified using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are washed before confirmation if needed.
