Typically, 1.5 to 2 inches of hair from the head or body is required. Hair testing options include analyzing for drugs, alcohol/ETG, or other tailored testing requirements.

In recent trends, hair follicle drug testing has been increasingly incorporated into drug screening methods. A multitude of employers, the judicial system, and professionals dealing with substance abuse are now favoring hair sample tests over urine tests. Employers with zero-tolerance drug policies, courts, and probation cases frequently rely on hair follicle tests. The advantage of hair testing is its extended detection window of up to 90 days, collected under strict supervision, making it very difficult to tamper with. Nonetheless, for identifying drug use within the previous 5 days, a urine test remains the most precise.

What it is, How it works

Hair analysis has gained recognition as an effective means for identifying drug and alcohol consumption. Hair captures a prolonged history of drug and alcohol usage by embedding biomarkers in the strands of growing hair. When retrieved near the scalp, hair can offer up to about a 3-month detection period for drugs and alcohol. Hair is easy to gather, hard to manipulate, and simple to transport.

A 1.5-inch segment containing roughly 200 hair strands (around the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal amount for both screening and verification. For EtG, additional tests, and/or panels above 10, 150mg of hair is advised. We suggest measuring the hair sample on a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be taken. References to head hair imply scalp hair only. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four core phases in processing a drug test result in a laboratory are Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample undergoes an initial process to enter a lab’s system. This process checks if the sample was sealed and transported correctly, allocates a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not included in an electronic chain of custody system.

Screening is a primary rapid test for drug abuse. Though Screening is an economical method to dismiss substance use in most cases, a positive screen must be confirmed to hold up in legal proceedings. Any cases with provisional positives in Screening necessitate a secondary verification.

If a test shows a provisional positive at the Screening stage, additional hair is taken from the original batch and prepared for Extraction. During this phase, the concentration of drugs is extracted from hair at a notably lower level compared to other methods (e.g., urine or oral fluid), which makes hair drug testing a challenging technique to execute.

Verification of any positive initial test result is done using GC/MS, GC/MS/MS, or LC/MS/MS. Every provisional positive is washed before confirmation as required. The entire lab process from Accessioning to Verification is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Extended detection span: Hair drug assessments can identify drug consumption for up to 90 days, unlike urine tests which cover a shorter duration.
  • Hard to manipulate: Altering a hair drug test result is very challenging, thus increasing result reliability.
  • Historical consumption insight: It can reveal a pattern of substance use across time, not merely recent intake.

Limitations:

  • Inability to pinpoint recent Intake: It usually takes about 5-7 days for drugs to become detectable within hair.
  • Expense: Hair drug analysis tends to be pricier compared to other testing methods.
  • Result variability: Elements like hair color and personal differences in hair growth can impact the levels of drug metabolites in hair.

Note: Often termed as "hair follicle tests", the assessment actually examines the hair strand rather than the hair follicle beneath the scalp
