A 1.5-inch segment containing roughly 200 hair strands (around the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal amount for both screening and verification. For EtG, additional tests, and/or panels above 10, 150mg of hair is advised. We suggest measuring the hair sample on a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be taken. References to head hair imply scalp hair only. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four core phases in processing a drug test result in a laboratory are Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample undergoes an initial process to enter a lab’s system. This process checks if the sample was sealed and transported correctly, allocates a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not included in an electronic chain of custody system.

Screening is a primary rapid test for drug abuse. Though Screening is an economical method to dismiss substance use in most cases, a positive screen must be confirmed to hold up in legal proceedings. Any cases with provisional positives in Screening necessitate a secondary verification.

If a test shows a provisional positive at the Screening stage, additional hair is taken from the original batch and prepared for Extraction. During this phase, the concentration of drugs is extracted from hair at a notably lower level compared to other methods (e.g., urine or oral fluid), which makes hair drug testing a challenging technique to execute.

Verification of any positive initial test result is done using GC/MS, GC/MS/MS, or LC/MS/MS. Every provisional positive is washed before confirmation as required. The entire lab process from Accessioning to Verification is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
