What it is, How it works

When sampled near the scalp, hair can reveal a detection period of up to three months for alcohol and drugs. It is easy to collect, tough to tamper with, and convenient to transport.

A sample of 1.5 inches from around 200 hair strands (similar in thickness to a #2 pencil) taken closest to the scalp provides approximately 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, or those exceeding 10 panels, 150mg of the sample is advisable. We suggest measuring the sample using a jeweler’s scale. If scalp hair isn't available, an equal amount of body hair can be used. When discussing head hair, we mean only scalp hair. Body hair encompasses all other types (e.g., facial, axillary, etc.).

Process Overview

The four principal steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is processed into a lab's system. This process includes checking that the sample was appropriately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and entering any supplementary data absent from an electronic chain of custody.

Screening is a preliminary quick check for illegal substances. While Screening is an efficient method for eliminating drug use in most samples, a positive result must be confirmed to be usable in legal settings. Samples showing presumptive positivity in Screening need a confirmatory test.

Should a sample be presumptive positive during Screening, additional hair is extracted from the original specimen for Extraction. In this phase, drugs are extracted from hair in much smaller concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to execute.

Positive results from Screening are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before Confirmation as necessary. The entire procedure from Accessioning to Confirmation is examined under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

Note: Although sometimes called "hair follicle tests", the test analyzes the hair strands and not the follicles beneath the scalp.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
