If scalp hair cannot be collected, an equivalent measure of body hair may be used. When we mention head hair, we are specifically referring to scalp hair. Body hair encompasses all other hair varieties (including facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial sample processing into a lab’s system. It involves ensuring the sample was sealed and shipped correctly, generating a unique LAN (Laboratory Accessioning Number), and completing any data input that isn’t provided by an electronic custody chain system.

Screening constitutes a preliminary examination for abused drugs. Although Screening is a cost-efficient method for excluding drug use in most cases, any positive screenings must be confirmed to be valid in legal contexts. Therefore, samples with presumptively positive results in Screening necessitate further confirmation.

If the Screening phase results in a presumptive positive, additional hair is extracted from the original sample and readied for Extraction. During this step, drugs are extracted from hair in significantly lower concentrations than other methods (such as urine or oral fluid), making hair drug analysis the most challenging approach to execute.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS or LC/MS/MS. All samples that test presumptive positive are washed before confirmation as necessary. The full lab process from Accessioning through to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and accreditation in accordance with ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
