For EtG add-ons, or tests exceeding 10 panels, a specimen of 150mg is suggested. It is advisable to measure the specimen using a jeweler’s scale. If scalp hair is not accessible, a comparable amount of body hair can be used. Note that 'head hair' strictly refers to hair on the scalp, whereas 'body hair' encompasses all other types (facial, axillary, etc.).

Process Overview

There are four primary steps in processing a drug test result in a laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is integrated into a lab’s system. This step includes confirming the sample's proper sealing and transportation, assigning a unique LAN (Laboratory Accessioning Number), and filling in any additional data not provided by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for the presence of drugs. Though Screening is a cost-efficient way to rule out drug use in most cases, any positive screening requires confirmation for legal validity. Presumptively positive samples in Screening must undergo further confirmation.

Should a sample be presumptively positive during Screening, additional hair from the original sample is used for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than in other methods (such as urine or oral fluid), making hair drug screening particularly challenging.

Confirmation for any positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples are cleaned as necessary prior to confirmation. The entire laboratory procedure from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
