When hair is taken close to the scalp, it can detect substances over a period of up to 3 months. Collecting hair is straightforward, relatively tamper-resistant, and convenient for shipping.

A section measuring 1.5 inches of approximately 200 hair strands (comparable in size to a #2 pencil) close to the scalp yields a 100mg hair sample, optimal for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is advisable. It is suggested to weigh the sample with a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair can be used. Mention of head hair specifically pertains to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The core steps in the laboratory analysis of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a lab’s system, ensuring it was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data logging not managed by an electronic chain of custody system.

Screening comprises a preliminary examination for drug abuse substances. Although Screening is economically efficient for ruling out drug use in most samples, a positive result necessitates confirmation to be court-admissible. Presumptively positive samples in Screening mandate further confirmation.

In cases of presumptive positives in the Screening phase, a portion of hair is retrieved from the beginning sample for Extraction. In this step, the extraction of drugs from hair occurs at a concentration lower than other methods (such as urine or oral fluid), highlighting hair drug screening as the most challenging procedure to conduct.

Confirmation of any positive screening is carried out using GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positives are cleansed prior to confirmation when needed. The laboratory operations from Accessioning to Confirmation undergo review conforming to both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.
