These tests suit companies with strict drug-free policies, probation requirements, and other needs. They offer a longer detection window, up to 90 days, with almost no possibility of tampering, although urine tests are more accurate for identifying recent drug use within the past 5 days.

What it is, How it works

Hair analysis has gained recognition as an effective means for identifying drug and alcohol consumption. Hair retains evidence of alcohol and drug use over time by capturing biomarkers within the strands as they grow. When sourced near the scalp, hair can reveal drug and alcohol usage for up to roughly three months. It is easy to gather, challenging to tamper with, and straightforward to transport.

A 1.5-inch section comprising about 200 hairs (approximately the diameter of a #2 pencil) taken closest to the scalp will yield 100mg of hair, the perfect sample amount for both screening and verification. For EtG, additional tests, and/or those exceeding 10 panels, a collection of 150mg is advised. We suggest measuring the sample weight using a jeweler’s scale. If scalp hair cannot be obtained, the equivalent volume of body hair can serve as a substitute. When we mention head hair, we refer solely to scalp hair, whereas body hair includes all other types (such as facial and underarm hair).

Process Overview

Four primary stages occur in the laboratory evaluation of a drug sample: Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the initial step in incorporating a sample into a laboratory’s framework. This entails checking that the sample is suitably sealed and transported, assigning it a random LAN (Laboratory Accessioning Number), and entering any data not captured by an electronic chain of custody system.

Screening provides a preliminary assessment for the presence of drugs. Though efficient for eliminating drug use in many samples, a positive outcome from the initial screen must be verified to hold up in legal settings. Any samples flagged as potentially positive at this stage require further confirmation.

Should a sample test potentially positive during Screening, more hair is retrieved from the original sample for Extraction. During this stage, drugs are drawn from hair at a much lower concentration level compared to other methods such as urine or oral fluid analysis, underscoring the complexity of hair drug analysis.

Verification of any affirmative screening results takes place through GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as potentially positive undergo washing before confirmation, as necessary. The comprehensive laboratory process, from Accessioning to Confirmation, adheres to both CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 criteria.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
