For EtG, additional tests, and/or those exceeding 10 panels, a specimen weight of 150mg is recommended. Utilizing a jeweler’s scale is advised for weighing the specimen. If scalp hair is unavailable, an equivalent amount of body hair may be gathered. Note that head hair refers solely to scalp hair, while body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing within a laboratory's framework. It encompasses verifying proper sealing and shipment, assigning a random Laboratory Accessioning Number (LAN), and completing any additional data entry not handled by an electronic chain of custody system.

Screening provides an initial rapid assessment for illegal drug use. Though it serves as an economical method to discount drug usage in most samples, any positive screen must be confirmed for court admissibility. Samples deemed presumptive positives in Screening require secondary confirmation.

In instances of presumptive positivity during Screening, additional hair is extracted from the initial specimen for Extraction. Within this phase, drugs are isolated from hair at significantly lower concentrations than other methods (such as urine or oral fluid samples), underscoring why hair drug screening poses unique challenges.

A positive screen confirmation involves GC/MS, GC/MS/MS, or LC/MS/MS methods. Before Confirmation, all presumptive positive samples undergo washing if required. This entire laboratory procedure, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair classification in conjunction with the ISO / IEC 17025 accreditation standards.
